Assistive Devices program (ADP) Authorizer - Aging Services

As an Assistive Devices Program (ADP) Authorizer with LifeMark’s Eldercare (long term care) and On The Go (retirement residences & seniors’ housing) division, you will have many opportunities to challenge yourself through applying a broad range of skills to your work with older adults. Effective communication with other members of the health care team (nurses, physicians, physiotherapists, dieticians, and pharmacists), residents and family members is imperative to becoming a part of our organization. Follow the guidelines of the College, ADP, and the residence you are servicing.

Responsibilities include:

  • Receiving the completed ADP referral form from the home and contacting the resident or designated decision maker
  • Conducting the assessment and completing the necessary follow up visits.
  • Ensuring all assessment findings and follow up visits are documented in the resident’s chart.
  • Completing follow up visits (as determined by yourself and the vendor) for suitability of trial equipment.
  • Completing and mailing the ADP application .

Qualifications:

  • Bachelors or Masters Degree in Occupational Therapy or Physiotherapy
  • Registration with the provincial Association (Occupational Therapy or Physiotherapy)
  • Registered Assistive Devices Program (ADP) Authorizer in the Ambulation Aids, Wheelchair and Positioning Devices/Seating categories.
