LifeMark’s Aging Services Division including our Eldercare (long term care) and On The Go (retirement & assistive living) brands, delivers rehabilitation and wellness services to older adults residing in residential care communities. We work together with the residents we serve to customize our programs to support and assist them to reach their potential in terms of function and quality of life. By setting mutually agreed upon goals, together we focus on their specific needs that typically include increasing balance, strength, coordination, ambulation and their ability to complete functional activities with minimal assistance.

As a key member of the interdisciplinary health care team you collaborate with all stakeholders to support residents’ goals. You do everything with integrity and not only appreciate but demonstrate that effective communication is paramount to the success of your treatment program.
